FRIMLEY Park Hospital lent a helping hand to very special guests from the second largest hospital in Singapore.
A group of 10 visitors from Tan Tock Seng Hospital visited to learn about innovations in health care and new models of care from the successful work within the ‘Happy, Healthy, at Home’ programme, while sharing their own experiences and positive outcomes as leader of Singapore’s health care system.
During the visit, the group explored Frimley Park Hospital, Aldershot Centre for Health and Farnham Centre for Health and learnt about our local integrated care and new models of care system which are being developed through the programme, which is a partnership between NHS North East Hampshire and Farnham Clinical Commissioning Group and eight health and social care organisations throughout North East Hampshire and Farnham.
The delegation visited the Aldershot Centre for Health to hear about positive impacts the programme is having across North East Hampshire and Farnham.The delegation then split into two groups: one visiting GP surgeries at Farnham Centre for Health to learn about integrated care teams and how GP practices are working together to deliver care within the community, and the other touring Frimley Park Hospital to see how its A&E and acute medical unit are using GPs to ensure patients are discharged at the right time with the right support services in place.
“We are challenged with balancing the needs of current day demands and the importance of implementing long-term health care strategies for our population,” Dr Eugene Fidelis Soh, chief executive officer of Tan Tock Seng Hospital, said.
“We recognise that other parts of the world are facing these challenges as well and we hope to learn how the success of others can help us deliver positive changes in our country.”
Dr Peter Bibawy, medical director at NHS North East Hampshire and Farnham Clinical Commissioning Group, said: “The Happy, Healthy, at Home programme aims to improve the health and well-being of local people, and the work we are doing is beginning to make a real, meaningful difference in our area.
“It is exciting to share our learning and successes with others around the world who are working toward the same goals and share our passion for working with local people to make positive changes within the health and social care system.”
